If so, let teachers know by uploading it to the 'Documents' section of your Dazzled listing.
If not, do both yourself and teachers a favour and create one. You'll be saving them work and give them one more reason to book with you.
If you're not sure how to go about it, we've put together some links to excursion policies and risk or hazard assessment forms for each state and territory. We couldn't find a link to a sample form for SA, NT, or WA (if you have one, please send it to us), but their policies list the type of information that should be included in a risk assessment, and you can always copy the basic format from one of the other states.
